One of my favourites is from the opening cutscene of Devil May Cry 4, which was famous for introducing second series protagonist Nero. In the opening cutscene we see Nero kicking demon ass in that ridiculous, over-the-top way Devil May Cry games are known for. What is less obvious about the opening cutscene is that Dante is actually watching Nero. You can see him on one of the surrounding buildings for a split second that is so short, pausing on the right frame is recommended.

One thing about RPGs is that I always watch the intros before the game starts. I found in all my years that you often miss something important. In Shining Force 2 and all of Final Fantasy games are like this. Same with Medevil.

The Wtiness has a mind blowing alternative ending before leaving the first area if you connect the white pillar closing the exit with the Sun

While that Dark Souls secret was not so "secret" if you ever were curious if it was possible to return to that asylum, it was still pretty surprising to run into Stray Demon for the first time.

When I played mw2 remastered when the say there’s Shepard I instantly domed him, not realizing there was a trophy involved.

Similar to the Gen. Shepard killing, you can snipe Makarov in the flash back mission for an achievement in MoW 1. Also, if you miss and hit Demitri (who is also in the keep) you will fail and get the “friendly fire will not be tolerated” prompt.
